Frail Care

Support for elderly individuals with physical limitations, ensuring safety, comfort, and dignity in their own homes.

Palliative Care

Specialized form of medical care focused on providing relief from the symptoms, pain, and stress of serious illness, regardless of the patient's age or stage of the illness. The primary goal of palliative care is to improve the quality of life for both the patient and their family, rather than attempting to cure the illness itself.

Terminal Care

Medical care and support provided to individuals who are in the final stages of a terminal illness—an illness that is expected to lead to death within a relatively short time, typically six months or less. The focus of terminal care is on comfort and quality of life rather than attempting to cure the illness. It involves managing symptoms, alleviating pain, and providing emotional, psychological, and spiritual support for both the patient and their family.

How it works

Three easy steps will guide you through the process.

Is a very important part of the clinical process, and can help to create a personalized treatment plan for a patient. Frail care is for people who are no longer able to care for themselves and require a higher level of care. It can be provided in a facility or at home.

A nursing care plan (NCP) is a formal process that correctly identifies existing needs and recognizes a client’s potential needs or risks. Care plans provide a way of communication among nurses, their patients, and other healthcare providers to achieve healthcare outcomes. Without the nursing care planning process, the quality and consistency of patient care would be lost.

The starting point of a care journey plan is typically an assessment of the patient's needs and health information. This assessment can include:

Reviewing patient history: This includes the patient's diagnosis, lab results, and medications.

Collecting health information: This includes collecting, validating, categorizing, summarizing, and interpreting the patient's health information.

Identifying needs: This involves identifying the patient's specific needs and where they need more support

At home Blood drawing

An at-home blood draw is a blood test that takes place in your home, where a certified phlebotomist collects a blood sample and sends it to a lab for analysis. The process is similar to a blood draw at a doctor's office or lab, but you can stay at home.
